Simply print the forms, fill them out, and bring them to your first appointment. This will help expedite the process, so you can spend less time in our office and more time enjoying your smile. We don’t want anything to stand in the way of your oral health, including finances. We accept a variety of payment methods, such as cash, checks, and credit cards (American Express, Discover, MasterCard, and Visa). If you’d like to make monthly payments, we can help you set up an account with CareCredit or Wells Fargo.
