New Patient Forms
Your time is valuable to us, and we want your visit to be as hassle-free as possible, which is why we’re happy to have our new patient forms available online. Simply download, fill out the forms, save and email back or print and bring them to your first appointment. This will help expedite the process so you can spend less time in our office and more time enjoying your smile. Of course, you also have the option of completing your forms when you arrive.

As a courtesy to our other patients, we kindly request 2 (two) business days’ notice when rescheduling appointments.
We Will Work With Your Insurance
If you currently have a dental insurance plan, we will gladly file your claims for you and work directly with your provider to maximize your coverage. We’ll do everything to make the process of using your benefits as easy and simple as possible.
Payments & Financing
We don’t want anything to stand in the way of your oral health, including finances. We accept a variety of payment methods, such as cash, checks, and credit cards (American Express, Discover, MasterCard, and Visa).

No Insurance? No problem!
Cornerstone Dental Arts has a Patient Loyalty Membership Plan that can help make dental care more affordable for those patients that do not have any dental insurance. It is quick and easy to sign up, and there is no waiting period, no prequalification, no limitations, and no exclusions.
